Unable to activate CityTV Plus streaming

I have been trying to active my cityTV Plus streaming since early Dec. I have done the process before without issue but now ...I go to CityTV Plus/activate choose my provider, confirm the code, when I click my Bell provider a page loads that says error instead of my Bell sign in page. CityTV Plus says its a provider issue.

I have attached a screenshot.
