Thank you for the question. I had to do a quick search to determine the exact plate cover you were looking for. As Bell no longer actively supports copper wireline service, including jacks, it is very, very unlikely they can supply the item you are looking for. You might try posting through social media or on searching eBay to see if there is a retired employee who might have some stashed away or a telephone equipment collector who would sell you what you need. I also see that there are a number of manufacturers of the plates out there. Perhaps look for a different supplier for the item you need. You could also replace the jack itself, if you feel comfortable doing so.

The jacks and plates that you mention have not been manufacturered by Northern Telecom since 1997. Northern Telecom became Nortel & eventually declared bankruptcy in 2009.
If your community has a surplus store, such as an Army Surplus, you can pretty well find just about anything there. You may also check to see if your community has a store that salvages hardware, etc. from bankruptcy’s & home demolitions.
It may be a lot easier & more convenient for you to just replace the entire jack & plate. Most communication jacks and plates sold today have standard hole alignments to fit drywall / plaster rings & outlet boxes that were installed years ago.
